Observed blade of can opener and holster unclean with accumulation of debris. PIC stated that the can opener and holster are cleaned weekly. Discussed with PIC that the can opener blade and holster must be cleaned after every use. Observed 7 knives and 1 spatula stored as clean with water spots on them. Observed spoons and forks stored as clean with water spots on the handle and the food contact surface of the spoon. CA: 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf. COS: All utensils and equipment mentioned above were placed in the 3-compartment sink to be washed, rinsed, and sanitized.

Observed urns holding iced tea unclean on the outside with accumulation of sticky residue and dust. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Discussed with PIC that the urns will be cleaned that night.

Observed fresh cut lettuce ready to eat salad bowl placed on top of raw bacon stored in box. Informed PIC that raw animal product must be store below ready to eat food and separated from ready to eat food to prevent cross contamination from raw food. PIC remove the salad and place on top of rack in RIC. COS

Observed unauthorized personnel were observed accessing the kitchen. Person in charge was not unable to demonstrate proper knowledge of food safety. ServSafe class scheduled for March 12th at Grace Nutrition Consulting, LLC.

At the time of inspection handwashing sink was blocked by mop, mop bucket and folder boxes. Informed PIC that hand sink should be accessible at all times for employee use. COS

Observed equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. PIC stated that the sink is not used. At the time of inspection dirty dishes were in the rinse sink section. Informed PIC of the proper use of the(3) compartment sink (wash, rinse and sanitizer.

Observed hole in ceiling, back door with light appear underneath. Informed PIC to contact contractor to repair to prevent pest entrance.
