Observed expired CSFM posted. CA:A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment. Discussed with PIC that they will schedule to retake the test for an updated CSFM.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Observed a container of sugar stored on the floor next to the back door. CA: Food shall be protected from contamination by storing it at least 6 inches off of the floor. COS: PIC moved sugar container to a surface that is more than 6 inches off the floor.

Observed light shining though gaps from the bottom, right side and top of back door of facility. CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by Solid, self-closing, tight-fitting doors. PIC mentioned that they will contact the owner so that they may provide weather strips or another form of closing the gaps on the back door.

Observed two bugs on the wall near the back door of the facility. CA: he presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by Routinely inspecting the premises for evidence of pests;; Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule. Discussed with PIC that they will contact pest control for their facility, because the building does not provide documentation of their Pest control methods.

Observed in use utensils being stored in water that was 79F. CA: During pauses between food preparation and dispensing, utensils must be kept in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi). COS: Employees emptied container and refilled container with hot water. Temperature measured at F.

Observed sugar container and espresso bean container with single service cup without handle stored inside for scooping. CA: During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar or coffee beans. COS: PIC removed single service cups from containers and provided each container with a scoop that has a handle; with care that the handle was not in contact with the foods.
