Observed no paper towel accessible at the only hand washing station in the main kitchen. CA: Advised PIC Each handwashing sink or group of adjacent handwashing sinks shall be provided with:

A heated-air hand drying device; Pf or

A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. COS: PIC replaced the paper towel during time of inspection

Observed no hand soap accessible at the only hand sink in the main kitchen due to dispenser batteries not working. CA: Advised PIC Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: PIC provided soap
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wiping cloths with food debris stored on the prep table. CA: Advised PIC Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). COS: PIC relocated the wiping cloths during time of inspection

Regulation: 511-6-1.05(6)(b) - cutting surfaces (c)
Observed the prep top cooler cutting board stained. CA: Advised PIC Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed dust build-up on the fan inside the walk-in cooler. CA: Advised PIC Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed the floors underneath the racks inside the walk-in cooler unclean with food debris. CA: Advised PIC Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(e) - floor covering, mats & duckboards (c)
Upon entering the kitchen observed a cardboard box stored in front of the grill/fryer used as a temporary mat and 1 piece of cardboard used to line a rack of seasonings. CA: Advised PIC mats and duckboards shall be designed to be removable and easily cleanable. COS: Mat was removed and discarded during time of inspection
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee food stored on the prep table upon entering the kitchen stored next to a bowl of sauce and other facility related items. CA: Advised PIC Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: PIC relocated the items during time of inspection and sanitized the table
17D - adequate ventilation and lighting; designated areas used

Observed employee cell phone and glasses stored on the prep table while actively preparing food. CA: Advised PIC Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: PIC removed items and sanitized the prep table during time of inspection
