Observed raw pork tenderloin above cut onions and raw eggs, and raw sausage above raw eggs in reach in cooler of grill prep area. CA: Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from ready-to-eat and other raw food based on final cook temperatures. COS: PIC moved onions to top of cooler and eggs and pork tenderloin under onions and moved sausage to the walk in cooler.

Observed the following foods stored uncovered: bacon, chicken, pork tenderloin stored in containers on stove; cheese in ice bath, 2 gravies and grits in steam table of front service area; raw chicken, pancake batter, raw steak, and cut onion in reach in cooler; 2 flours on table in back main kitchen area. CA: Food must be stored covered COS: Manager covered foods

Observed black soil residue on lid and walls inside ice bin in front service area. Observed brown reside buildup in side ice machine on lid, panel and walls. CA: Food contact surfaces must be cleaned to sight and touch. COS: Employee cleaned inside ice bin and ice machine

Observed current CFSM certificate not posted in public view. CA: CFSM certificate must be posted in public view.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed build-up of excess grease and food debris on floors, behind and under cooking equipment and inside cabinets under the counter in front service area, observed black residue/food debris along base boards, walls, door frames and under shelving and equipment in main kitchen. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean

Observed gasket seal around back door of main kitchen with cracks/gaps between door and door frame allowing sunlight through. CA: Doors must be installed tightly sealed without gaps/openings
