Observed pizza dough stored in the reach in cooler under the prep top on baking sheet with no covering.
CA: Employee moved pizza dough to a covered container.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed various seasonings that have been removed from original container not labeled with the working name of the product. CA: PIC will label all bottles with common name. working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.
18 - insects, rodents, and animals not present

Observed door gap on back door with door sweep big enough for insect to enter the facility.
RCA: Adjust door to seal opening to outside section of restaurant and readjust the sweep to ensure it covers the entire door gap at the bottom.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed debris and grease buildup on ceiling tiles throughout the facility.
RCA: Clean ceiling regularly to eliminate chance of contamination.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee personal items stored above and next to food items (lunchbox, water bottle, soda can, etc.) and prep counter. CA: PIC moved employee items to designated area away from food items
