Observed that the facility did not have a food safety manager certification posted. CA: The facility shall have a certified food safety manager in its employ that is designated solely to this location and that certification shall be posted in public view.
2-2B - proper eating, tasting, drinking, or tobacco use
Regulation: 511-6-1.03(5)(k)1&2 - eating, drinking, or using tobacco (c)
Observed employee drinks stored in non-single-use bottles that were stored with and above food and food contact surfaces. COS. Removed. CA: Employee drinks shall be stored in single-use containers with a lid and a straw and shall be handled to prevent contamination.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed accumulation of mold/mildew-like substance on the inside of the soda nozzles. COS. Soda nozzles were removed to be washed, rinsed, and sanitized. CA: Food contact surfaces shall be clean to sight and touch.

Observed that the facility was thawing food in stagnant water. COS. Removed. CA: Thawing shall be accomplished using approved means such as under cold running water, by placing under refrigeration, by cooking from frozen, or by microwaving and immediately serving.
12B - personal cleanliness

Observed employee wearing jewelry (bracelets and watches) while preparing food. COS. Removed. CA: With the exception of a plain band ring, no jewelry shall be worn on the hands or wrists while preparing food.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloth stored on counter. COS. Placed into sanitizer solution. CA: Wet wiping cloths shall be stored in sanitizer solution that is at the appropriate concentration between uses.

Observed accumulation of debris on the inside of equipment. CA: Equipment shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.

Observed accumulation of debris on floors, particularly in the walk-in's. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.

Observed that the facility had and older score of 100 'A' posted instead of their most recent score 74 'C'. CA: The most recent inspection report shall be posted within 15 feet of the front door, 5-7 feet from the floor, readable at a distance of 1 foot away, and in a conspicuous place.

Observed that the permit was not posted. CA: The food service permit shall be posted in public view.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed that the facility was storing utensils in not hot, stagnant water. COS. Removed. CA: In-use utensils shall be stored using approved means such as in hot (135 F) water, in running water, in the product with the handle above the level of the product, or in/on a clean container/surface.
14B - utensils, equipment and linens: properly stored, dried, handled
