2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed that all three of the hand sinks in the facility were not accessible; the one upfront was being used to store single-use items, the one in the middle of the kitchen had trash in it and was blocked by a freezer and chair, and the one in the back was blocked by a trash can and a box of trash. COS. All items removed. CA: Hand sinks shall always be kept clear and accessible.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed that the rear door was not self-closing and that the there was a large gap under the door. Recommended action: For the gap under the door, a rubber weather stripping is in place, but the floor is uneven, causing there to be gaps under it; recommend considering a bristle door sweep or other similar item that may be able to work despite the uneven ground. CA: Exterior openings shall be self-closing and tight-fitting.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(2)(l) - toilet rooms, enclosed (c)
Observed that the bathroom doors were open/not self-closing. CA: Bathroom doors shall be self-closing and tight-fitting.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed heavy accumulation of grease and debris on the floors, walls, and ceilings throughout the facility. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.
