Observed that the facility is keeping raw fish that is in reduced oxygen packaging thawed in refrigeration. COS. Discarded. CA: Fish stored in a reduced oxygen packaging shall be removed from the packaging prior to or immediately upon completion of thawing.

Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed the back door did not have a self-closure preventing it from closing on its own. CA: Exterior openings shall be self-closing and tight fitting.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris and standing water on the floors throughout the facility. CA: Facility shall be clean and cleaning frequency shall be enough to prevent contamination.

Observed that the facility is storing food service items in the unfinished shed outside. CA: Floors, walls, and ceilings shall be constructed of appropriate materials for the usage of the space and shall be easily cleanable.

Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee belongings (phones) stored next to clean equipment. COS. Phones removed from kitchen. CA: Employee belongings shall be stored and handled in a manner that prevents contamination.
