Observed reduced oxygen packaged fish that is labeled to be kept frozen thawing in the original packaging. COS. Removed from packaging. Code: Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment prior to its thawing under refrigeration.

Observed accumulation of debris and food on floors throughout the facility especially under equipment and shelving. Code: Facility shall be clean and cleaning frequency shall be enough to prevent accumulation.
