Observed employee drinks in tumblers stored with and above food both in the main kitchen and in the front food service area. COS. Removed. Code: Employee drinks shall be in single-use containers with a lid and a straw and shall be handled to prevent contmination.

Observed that the hand sink in the front food service area does not have hand cleanser. Code: All hand sinks shall be stocked with hand cleanser.

Observed noneasily identifiable foods in dry storage container and squirt bottles on the line and dry storage containers in the front food service area that were not labeled with the common name of the product. COS. Labeled. Code: All containers of non-easily identifiable foods shall be labeled with the common name of the product inside.

Observed that the facility has a written notification of the major food allergens used in the facility, however, it does not cover all of the allergens used in the facility (shellfish is missing). Code: Facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.
12D - washing fruits and vegetables

Observed that the facility is thawing raw shrimp in the vegetable sink. COS. Shrimp were moved and sink was washed, rinsed, and sanitized. Code: Vegetables, before use, shall be washed in a sink designated for that purpose only.

Observed that the rear exterior door is not tight-fitting after self-closing. Code: Exterior doors shall be self-closing and tight-fitting.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris and food on the floors in the kitchen, particularly in the walk-in cooler. Code: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.
