Observed dish soap on the 3-compartment sink not labeled, pump bottles hand-washing soap being stored on top of a deep freezer, and container of sanitizer tablets were being stored next to clean dishes. COS: sanitizer tablets and handwashing soaps were removed. Code: Poisonous or toxics chemicals shall be stored in an area that is not above food, equipment, utensils, linens, and single-service or single-use article

Observed food handlers wearing multiple bracelets while working with food and drinks. COS removed. Code: With the exception of a plain band ring, no jewelry may be worn on the hands or wrists to include medical bracelets.

Observed wet wiping cloths being stored on food contact surface and not in sanitizer buckets. COS: Wet wiping cloths placed into sanitizer bucket. Code: Wet wiping cloths shall be stored in sanitizer solution in between uses.

Observed single service items being stored under HVAC/mechanical system with no protection. Code: single service items shall not be stored under any sources of contamination.

Observed the wall to floor junctions in the restrooms did not have any coving. Code: Floor and Wall Junctures shall be coved, enclosed or sealed.

Observed employee personal items being stored on top of deep freezer as well as with dry storage. COS. Items were moved. Code: Employee belongings shall be stored to prevent contamination.

Observed that one of the hand sinks was being used as a dump sink. COS. PIC informed employees that hand sinks are for hand washing only. CA: Hand wash sinks are for hand washing only.

Observed the back door's self closure was not properly attached causing the back door not to self-close. CA: Exterior doors shall be self-closing and tight fitting.

Observed boxes of single use items stored directly on the floor. COS. Items raised off floor. CA: Single-use items shall be stored 6 inches off the floor.

Observed single-use containers stored upright. COS. Containers inverted. CA: Single-use items shall be stored to prevent contamination.

Observed the hand sink in the front food service area was not stocked with a hand cleanser. COS. Soap placed at sink. CA: Hand cleanser shall be provided at all hand sinks.

Observed containers of coffee stored directly on the floor in the walk-in cooler. COS. Containers raised 6 inches off the ground. CA: Food shall be stored at least 6 inches off the ground to prevent contamination.

Observed single-use utensils stored where the food-contact surfaces is upright. COS. All utensils inverted. CA: Single-use utensils shall be handled, displayed, and dispensed so that contamination of the food-contact surface is prevented.

Observed that the permit was not displayed in public view. CA: Food service permit shall be displayed in public view.

Observed single-use items stored upright without overhead protection. CA: Single-use items shall be stored inverted or covered to protect from overhead contamination.

Observed employee belongings stored with and above food. COS. Moved. CA: Employee belongings shall be stored to prevent contamination.

Observed cold TCS food that was held above 41 F (ham, egg, milk). COS. Items were discarded by the PIC. CA: TCS food that uses cold holding as a control for safety shall be held at 41 F or below.

Observed that one of the uni-sex restrooms did not have a covered trash receptacle. CA: All restrooms used by women shall have a covered trash receptacle.
