Regulation: 511-6-1.07(6)(c) - storage, separation (p)
Observed dish soap on the 3-compartment sink not labeled, pump bottles hand-washing soap being stored on top of a deep freezer, and container of sanitizer tablets were being stored next to clean dishes. COS: sanitizer tablets and handwashing soaps were removed. Code: Poisonous or toxics chemicals shall be stored in an area that is not above food, equipment, utensils, linens, and single-service or single-use article
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed food handlers wearing multiple bracelets while working with food and drinks. COS removed. Code: With the exception of a plain band ring, no jewelry may be worn on the hands or wrists to include medical bracelets.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloths being stored on food contact surface and not in sanitizer buckets. COS: Wet wiping cloths placed into sanitizer bucket. Code: Wet wiping cloths shall be stored in sanitizer solution in between uses.

Regulation: 511-6-1.05(10)(f) - prohibitions; single service and single use articles (c)
Observed single service items being stored under HVAC/mechanical system with no protection. Code: single service items shall not be stored under any sources of contamination.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(c) - floor and wall junctures, coved, & enclosed or sealed, water-flushed floors provided with drains & graded to drain (c)
Observed the wall to floor junctions in the restrooms did not have any coving. Code: Floor and Wall Junctures shall be coved, enclosed or sealed.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee personal items being stored on top of deep freezer as well as with dry storage. COS. Items were moved. Code: Employee belongings shall be stored to prevent contamination.
