Observed that the facility did not have written disclosure of the major food allergens used in the facility. CA: The facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.

Observed wet wiping cloths stored on counter. COS. Sanitizer bucket was made and wet wiping cloths placed in to sanitizer bucket. CA: Wet wiping cloths shall be stored in sanitizer solution at the appropriate concentration between uses.

Observed the back door had gaps at the bottom. CA: Exterior openings shall be self-closing and tight fitting.

Observed food stored uncovered. COS. Lids placed. CA: Food shall be stored covered.
