FLOYD BOWLING AND AMUSEMENT

Observed multiple stacks of sliced cheese removed from their original packaging and placed into a sealed plastic bag with a date mark of 4/19/26. CA: TCS foods must be discarded after 7 days. COS: The CFSM discarded the cheese.

Observed a Styrofoam cup and a small plastic container without a handle stored directly in a container of sugar in the dry storage area. CA: Scoops must be stored with the handle above the food line. COS: Both items were removed from the sugar.

Observed multiple boxes of single-use cups stored directly on the floor in the dry storage area. CA: Utensils must be stored at least 6 inches off of the floor. COS: The utensils were moved onto a shelf.

Improper disposition of date marked, potentially hazardous TCS foods observed. Observed several opened packages of sliced cheese in reach in coolers held beyond 7 day expiration date. All expired cheese dated 10/17/25. Corrective Action: CFSM was notified, all expired date marked food was discarded. Employees were educated on proper date marking and disposition of food items after 7 days.

Observed multiple working containers of sauces removed from the original container and not identified by a common name. CA: Discussed ensuring all working containers of food are properly labeled and date marked with PIC. PIC properly labeled all working containers with their common names.

Observed employee cell phone and other personal items store in food prep area. CA: Discussed storage of personal items with PIC. PIC will designate an area for storage of employee personal items.
