Observed during the course of this inspection that the men's restroom handwashing sink could not provide warm water. CA: Glynn EH instructs management to have the sink repaired and able to provide at least 85 deg f warm water.

Observed during the course of this inspection that the "Tea Urn" spouts displayed old drink buildup inside of them. COS: Glynn EH instructed management to have the urns replaced with clean urns before every event. Management complied. CA: Glynn EH instructs management to ensure that all food/drink contact surfaces are kept clean to sight and touch as well as sanitary.

Observed an unlabeled container of flour at this location at the time of this inspection. COS Discarded CA Reeducate staff on proper labeling of products being stored out of their original containers.

Water and moldlike substance were found in a RI cooler at this location at the time of this inspection. CA Clean up called in.

Observed rodent droppings near the wall in the room with the walk in cooler and walk in freezer at the time of this inspection. CA: Facility must ensure the facility is protected from the entry of rodents by ensuring all doors have a tight-fitting seal. Three exterior doorways were observed with a gap under the door

Floors in the prep area and between the WI cooler and freezer were sticky andhad grease and food build up at the time of this inspection. CA Stewarding will fix.

Observed damaged hood system filters and a gap between filters at the time of this inspection. CA: Facility must repair/replace hood filters to ensure proper function.

REPEAT: Observed rodent droppings in the room between the walk in freezer and walk in cooler. Also found a dead rodent in the well house at this facility. CA: Facility will continue to better seal the building against pests and work with pest control.

Bottom seals on outside screen doors were found damaged and the screen on one door was found torn at the time of this inspection. CA MGR stated it would be repaired.

The screen covering the exhaust fan on the roof of the building was missing at the time of this inspection. CA Maint was called to repair the issue.

Containers of cooking oil were found stored on the floor by the fryers at the time of this inspection. CA Mgr stated it would be moved to a proper storage location.

REPEAT: Observed rodent droppings in the room between the walk in freezer and walk in cooler. Also found a dead rodent in the well house at this facility. CA: Facility will continue to better seal the building against pests and work with pest control.

Ice buckets were found stored not inverted, exposed to vertical contamination at the time of this inspection. CA MGr stated they would be flipped over.

Found a melted cake mess in the WI cooler, a huge ice leak in the WI Freezer, mold on the outer seal of RI cooler 2, and a broken water faucet in the screened ding area at the time of this inspection. CA Mgr stated all would be repaire.

Observed at the time of this inspection prep sink faucet leaking and hand sink handle leaking when on. CA: Director stated repairs will be made to plumbing system.
