Utensils not being stored handle grab up. They were observed being stored with mouth portion up which when you grabbed them cross contamination would happen. Need to be stored properly inverted in the container.

Observed Staff chopping salad lettuce with bare hands (not wearing gloves). Employees prepping food should wash hands and then put on gloves prior to working with Ready to Eat food product. Staff left station went to front of house the came back and observed her wearing gloves and hat. Notified PIC of action to correct

Large Salad Reach In Cooler was observed not maintaining proper cold hold temps of 41 degrees and below. Temps were reading bulk hot dogs 48, salad mix individuals several at 46 degrees. Staff called maintenance for repair and discarded food out of temp.
