6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Observed to-go containers of coleslaw with no datemarking. PIC stated they were from last Friday.
CA: A food that requires datemarking shall be discarded if it exceeds 7 days or is in a container or package that does not bear a date or day.
COS: PIC discarded coleslaw containers as 7 days were exceeded.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(b) - working containers, common name (pf)
Observed two spray bottles of clear liquid substance handing on wire shelf in main kitchen. PIC stated they were disinfectant.
CA: Working containers used for storing toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
COS: PIC relabeled with common name.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(c) - storage, separation (p)
Observed two spray bottles containing disinfecting store hanging above clean equipment.
CA: Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles.
COS: Spray bottles were moved over so they were not above clean equipment.
10D - food properly labeled; original container
Regulation: 511-6-1.04(8)(b) - expired foods (p)
Observed 2 containers of store-bought coleslaw with an expiration date of with 12/19/2025 in reach in cooler in main kitchen area.
CA: Time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date.
COS: PIC discarded containers of coleslaw.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed the initial inspection report posted in compliance (hung on wall) but display of most current inspection report not (sitting on table).
CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Health dept. to check back by correct by date

14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed scoops stored in dry flour and batter mix with their handles in the product.
CA: During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in the food with their handles above the top of the food and the container.
COS: Scoops were stored with their handles up.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed heavy accumulation of grease and debris on floors around grill and fryers.
CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
Health dept to check back once deep cleaning is done.

17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee cellphone on prep top surface in main kitchen.
CA: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.
COS: Employee removed cellphone.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.05(3)(d) - ventilation hood system, adequacy, adequate to prevent grease & condensation build-up (c)
Observed hood vents above grill and fryer with a heavy accumulation of dust and grease buildup.
CA: Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings.
Health dept. to check back once deep cleaning/service is done.
