Observed heavy black buildup inside bulk ice machine in kitchen. C/A: Clean. Increase cleaning frequency.

Observed blue cheese and sliced tomatoes held past the date marked time limits in the coolers. COS- food was discarded C/A: Discard. Monitor use-by dates.

Observed employees' drinks stored next to tortilla chips on table in kitchen. C/A: Store employees' drinks in a designated away from food prep areas.

Observed several employees actively preparing food with necklaces and a bracelet. C/A: Only a plain band can be worn during food prep.
