Observed no evidence of Employee Health Reporting Agreements and person in charge was unable to demonstrate knowledge of the reportable symptoms of foodborne illnesses. C/A: Maintain signed agreements on-site and ensure staff is trained on reporting requirements.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(c) - storage, separation (p)
Observed several chemicals (bleach, disinfectants, and Purell sanitizer) stored on racks holding corn syrups. C/A: Do not store chemicals next to food products.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed accumulations of dust and debris on the compressor fans and inside the coolers in both the bar and kitchen area. C/A: Clean and sanitize all cooler components to prevent contamination.

Observed hole in ceiling and debris on floor in back kitchen area. C/A: Repair to limit access to pests. Increase cleaning frequency.
