Observed no certified food safety manager certificate posted for public viewing. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. PIC to post the food safety certification for public viewing.
2-2A - management knowledge, responsibilities, reporting

Observed no written employee health policy present and PIC not able to verbalize employee health policy. Explaining the responsibility of the person in charge for preventing the transmission of food borne disease by a food employee who has a disease or medical condition that may cause foodborne disease. Describing the symptoms associated with the diseases that are transmissible through food. Explaining how the person in charge , food employees, and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees.

Observed employee health acknowledgement form not present. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
2-2E - response procedures for vomiting & diarrheal events

Observed vomit and diarrhea cleanup procedures not available. A food service establishment shall have written procedures for employees to follow and an EPA registered disinfectant available against Norovirus available on site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment.

Observed entry / exit door self closer in need of repair. Entry/exit doors that open to the outside must be self closing, tight - fitting or protected by other approved methods
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed water system plumbing (by way of verbal notification from operator) and also by water valves not on, in need of repair. A plumbing system shall be repaired according to law and maintained in good repair.
