CORNMEAL IS STORED IN THE MAIN KITCHEN BY THE PIZZA OVEN IN A PLASTIC RUBBERMAID BUCKET WITH LID. THE CONTAINER IS BROKEN IN MANY PLACES, WITH A THICK COATING OF GRIME/GREASE/DEBRIS COATING THE ENTIRE CONTAINER. THIS CONTAINER MUST BE DISCARDED IMMEDIATELY AND REPLACED WITH A SUITABLE FOOD SAFE CONTAINER. ALL EQUIPMENT AND UTENSILS WHICH CONTACTS FOOD MUST BE CLEANED AND SANITIZED REGULARLY, AND KEPT FROM ANY BUILDUP OF GRIME OR DEBRIS.

DISCLAIMER MUST BE ADDED TO THE MENU, AND EXISTING ALLERGEN NOTIFICATION MUST BE MODIFIED. ALLERGEN NOTIFICATION MUST SPECIFICALLY LIST WHICH OF THE 9 MAJOR FOOD ALLERGENS ARE SERVED IN THE ESTABLISHMENT, AND ALL STAFF MUST BE TRAINED ON CROSS-CONTAMINATION PREVENTION AND SYMPTOMS OF ANAPHYLACTIC SHOCK.
