Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employee preparing sandwiches is wearing a watch.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up observed in reach-in freezer by mop sink.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of conveyor toaster oven has accumulation of food debris.

Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Wiping cloth solution at front counter tested at 0ppm quaternary ammonium. Chemical line found not dispensing chemical. Operator remade solution with chlorine and tested at 50ppm. Establishment will utilize chlorine until repair can be made. Operator called for service during inspection. Corrective Action Taken

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Slicer has dried food debris. Per operator, slicer not used today. Employee cleaned slicer during inspection. Toaster conveyor has debris build up. Corrected On-Site

Basic - Floor/wall/ceiling/vents soiled/has accumulation of debris. Wall behind stand up reach in drink cooler has excessive dust. Wall around/under handwash sink behind front counter is soiled.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Counter area under lobby soda machine and underplating around soda nozzles are soiled.
