High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed cook remove gloves put hands on hips then put on new gloves no hand wash. Discussed proper hand washing practices with Operator. Corrective Action Taken

Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 6 pieces of Mahi still in commercially sealed rop completely thawed. See Stop Sale. Provided Operator with memo regarding proper thawing of commercially packaged fish.
