Observed stains/holes on mattress covers/ pads/ comforter (102) in rooms: 127, 129,135,229,227, 301,300. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed stairs in need of repair and observed hole in walkway ceiling overhang in several areas near stairs. Grounds area and exterior of building shall be maintained in good condition

Observed rust in the tubs of multiple rooms 127, 135. Observed hole in the tiles in 139, 227. Observed fans not working 300,and 301. Observed crack in the sink in 329. Rooms shall be maintained in good condition. Advised to make repairs

Observed rips/holes, tears, or stains on draperies in rooms: 102,130, 227 and 346. Advised PIC all draperies must be maintained in good repair, clean, and free of vermin

Observed stains/holes on mattress covers/ pads/ comforter (102) in rooms: 104,129, 231, 229, 346, 342 and 300. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed stains on mattress cover/ sheets in rooms: 106, 227 (hairs), 216, 218, 333, 337, 341. Advised PIC Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed debris on shower head in rooms 231, 227, 218, 337, 341; observed chipped floors tiles and floors with excessive debris in rooms 127, 231, 227, 333, 337, 341; Observed stains/ cracks on toilet seat covers in rooms 218, 231. Advised PIC Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed chemical bottles unlabeled on the housekeeping cart. Advised PIC all chemicals shall be labeled with the common name on the container

Observed stains on pillow and minor stains on mattress (room 104); stain on boxspring (room 245); stain on mattress pad (room 210); staining on mattress pad (room 208, 204, 300, and 301). Advised PIC Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed debris on showerhead (room 104); minor stains on bathroom ceiling (room 106), stains on bathroom wall (room 105), missing light shield cover (room 243), stain on bathroom light fixture (room 245), rusting on showerhead and bathtub drain (room 204). Advised PIC Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed visible light entry at entry door for rooms: 104, 106, 243, 245, 210, 204, 300, and 301. Advised PIC Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed debris and residue on interior food contact surface area of ice machine. Advised PIC that machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed debris in dresser drawers in rooms 102, 104, 201. 210, and 300; observed stains on lamp shade in rooms 102 and 301; observed stains on chair in rooms 110, 204, and 300; observed food debris and rusting in microwave in rooms 201, 105, 204, 210, 212, and 30; observed stains on couch cushions in room 301. 300 and 201; observed stains on curtain in rooms 105, 210, and 201; observed residue in refrigerator in room 300. Advised PIC that all furniture and accessories shall be kept clean and in good repair

Observed stains on mattress, mattress cover, pillows, and linens in rooms 102, 104, 110, 201, 204, and 300. Advised PIC that all mattresses, mattress covers, pillow, and bed linens shall be kept clean at all times
