Observed stains on furniture and worn spots on multiple furniture items and damaged refrigerators in multiple rooms 127,139,301. Furniture shall be maintained in good condition
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed stains/holes on mattress covers/ pads/ comforter (102) in rooms: 127, 129,135,229,227, 301,300. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean
34 - grounds, walkways, porches and hallways maintained; drives provide unobstructed width and proper spacing; graded to drain; trailer spaces marked clearly, areas clean and maintained in good repair

Observed stairs in need of repair and observed hole in walkway ceiling overhang in several areas near stairs. Grounds area and exterior of building shall be maintained in good condition
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed rust in the tubs of multiple rooms 127, 135. Observed hole in the tiles in 139, 227. Observed fans not working 300,and 301. Observed crack in the sink in 329. Rooms shall be maintained in good condition. Advised to make repairs
