Observed no paper towels at the hand sinks in the main kitchen and bar. COS: PIC placed paper towels. CA: Advised PIC hand drying provisions shall be available at hand sinks at all times.

Observed clogged floor drain in the main kitchen area at the hand sink and leak at hand wash sink. Advised PIC to repair by 11/15/2025. CA: All plumbing systems shall be maintained in good repair at all times.

Observed ice scoop at bar area with scoop handle directly down in ice. COS: PIC moved scoop with handle up. CA: Properly store utensils.

Observed prep top cooler and reach in cooler in main kitchen in disrepair. Advised to repair. CA: Equipment shall be kept in good repair.

Observed buildup on equipment. Advised to clean. CA: Non-food contact surfaces shall be kept clean.

Observed walls and floors in need of cleaning. Advised to clean. CA: Floors, walls, and ceilings shall be kept clean.
