6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple TCS foods (raw catfish filet, raw shrimp, and raw catfish nuggets) at the prep top cooler cold holding above 41 degrees f. Advised PIC TCS foods shall be cold held at 41 degrees F. or below. COS: Had PIC move to WIC for proper cooling.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed wiping cloth stored inside the front handwash sink. Advised PIC all hand sinks shall be easily accessible at all times. COS: Had PIC remove wiping cloth from hand sink.

Regulation: 511-6-1.02(1)(f)(1) - permit, displayed (c)
Upon arrival, observed 2025 Food Service permit posted in public view. Advised PIC that a current Food Service permit shall be posted in public view. Advised to obtain 2026 permit and post in public view.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(q)1&3 - good repair & calibration (c)
Observed PIC in disrepair with an ambient of 44 degrees F. Advised PIC ambient air temperature shall be maintained in good repair and within the intended range.
