observed daylight at back door. CA: Discussed repairing door with weather stripping and sweep to prevent entry of insects, etc. with pic COS: pic will have repaired and health dept will re-check.

Observed three compartment sink not secured to the wall or floor in main kitchen. The back right leg was observed to be bent. Per PIC, the three compartment sink was recently replaced. Corrective Action: Discussed fixing the legs on the three compartment sink and properly securing and sealing it the wall and floor in main kitchen with PIC. The silver splash guard on the wall must be extended to cover the full length of the sink. This will be rechecked by the Health Department in 2 weeks.

Observed accumulation of water under the ice machine. Corrective Action: Discussed having ice machine serviced to determine cause and repair water leak with PIC. Until machine is serviced, a pan should be placed underneath to collect the dripping water. This will be rechecked by the Health Department in one week.
