Observed several food items held past time marked when using time as a public health control, including items hot-holding and sushi. Corrective Action: Discussed proper use of the time as a public health control procedure with PIC. All items held past time marked were discarded.

Observed person in charge not performing duties by ensuring all employees are properly trained and following food safety practices in the food service establishment. Observed food service employee changing tasks without removing gloves and properly washing hands. Observed food service employee washing serving utensils in handwashing sink at hibachi grill. Observed clean, dry pitchers in standing water next to handwashing sink. Observed raw mussels stored above French fries and open container of spring rolls. Observed designated produce sink being used for tasks other than washing fruits or vegetables, observed several dented cans that were removed from service and will provide documentation when sent back. Corrective Action: Educated PIC on food safety practices, including the use of gloves, when to wash hands, using the handwashing sink only to wash hands, proper storage of utensils and equipment to prevent contamination, proper separation of raw animal food items during storage, and using the designated produce sink for that purpose only. All food service employees will be re-trained on food safety regulations to ensure compliance with the food code.

Observed multiple live cockroaches on kitchen walls, shelving and inside a container of wonton chips. Corrective Action: PIC was notified. The plastic container of wonton chips was discarded. Discussed cleaning frequency to prevent pests and keeping food items covered at all times, including the water for the rice scoop. PIC will contact pest control to have this area properly treated. Documentation from pest control must be provided for review by the Health Department.
