Observed damaged concrete in main kitchen area with exposed tile underneath. CA: PIC is in contact with management and will have flooring repaired/patched.

Observed leak from three compartment sink puddling into floor. CA: PIC will have sink repaired and will ensure water drains into floor drain and will minimize the amount of water puddling.

Observed handwashing sink in ware washing area not operational. CFSM showed me a leak. No paper towels present at handwashing sink. Corrective action: CFSM was notified; handwashing sink leak to be fixed and paper towels stocked. Will perform follow-up on 01/27/2025. Performed informal follow-up on 02/20/2025. Back handwashing sink is operational.

Observed drink nozzles in drive-thru station and dine-in area heavily soiled. Corrective action: CFSM was notified; employee assigned to clean drink nozzles. Discussed increasing cleaning frequency and using small brushes to clean crevices.

Observed buildup on dish rack of clean dishes. Corrective action: CFSM notified; discussed purchasing hard plastic liners for the shelves. Performed informal follow-up on 02/20/25. Clean dish racks were still heavily soiled with grease and dust debris. I discussed with PIC what their remodeling plans are, and to send any information to me. Discussed if the remodel does not take place for several more months, the store is to buy a new rack or purchase hard plastic liners.
