Observed mold growth/debris build up in bulk ice machine. Corrective Action (CA): Person in Charge (PIC) instructed to ensure ice machine is cleaned out regularly to prevent mold growth/debris build up.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed lactic acid sanitizer concentration from three compartment sink dispenser below 704-1875 ppm range. CA: PIC instructed to ensure sanitizers are maintained at proper concentration. PIC instructed to switch to chlorine sanitizer until fixed.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(g) - ready-to-eat time/temperature control for safety food, date marking (pf)
Observed multiple time/temperature control for safety (TCS) ready to eat (RTE) items in walk-in cooler with no 7 day discard date. CA: PIC instructed to ensure all RTE TCS items are properly dated with 7 day discard date. Items dated. COS.

Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Sanitizer mixing system observed currently incapable to mix sanitizer to proper concentration. PIC instructed to call technician to adjust concentration to appropriate levels.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf)
No high temperature measuring device or lactic acid test strips present in establishment.
